Our mission is to come together to decide what’s best for you, utilizing any necessary outside services to meet your individual health needs as they relate to your hospital discharge (leaving the hospital).
Continuing Care Coordinator Hours
Monday-Friday, 8:00am - 5:00pm
During your stay at Parkview, a continuing care coordinator may meet with you to discuss any discharge planning needs. All continuing care coordinators have backgrounds in social work and/or nursing. As soon as you are admitted, an assessment is made regarding your potential needs.
Continuing care coordinators get involved with:
- Geriatrics (elderly)
- Your living situation
- Insurance benefits coordination
- Safety issues
- Need for in-home services
- Need for rehab/nursing home placements
- We will work with you, your physician and providers to develop an appropriate plan for your discharge.
- Home healthcare
- Private pay assistance
- Meals on Wheels
- Respite care
- Durable medical equipment
- Assisted living
- Support group referrals
Our goal is to make sure that you will have appropriate assistance to aid in your recovery. If you have any questions or concerns that you would like addressed, please ask your nurse to contact a Continuing Care Coordinator. We look forward to meeting and assisting you in a safe discharge.
