Welcome to Parkview Medical Center’s Patient Portal: My Care!

My Care

Become active in your own care and treatment with My Care. It is a free, convenient and secure online service that can help you access and manage your health and obtain information regarding your hospital services quickly.

My Care Proxy Authorization Form

My Care allows you the ability to be better informed and manage your health care by viewing the most recent:

  • Medical records with the ability to print, download and transmit
  • Test results and reports
  • Upcoming appointments
  • Medical conditions
  • Most recent hospital updated medications, allergies, etc.

My Care Hospital Enrollment

In order for you to enroll, you will need to speak to a hospital representative. An active email address will be required for this setup. After you've spoken to a hospital representative, you will receive an email like the one shown below:

    1. Click on the listed link.
    2. Enter your temporary Login ID provided in the email.
    3. Enter your one-time password provided in the email.

Next, you will need to personalize your account information:

Finally, you will be asked to logoff and return back into with the User ID and Password you successfully saved.

Self Enrollment from Home

To Self Enroll in My Care, you will first need to have some personal information ready. Be prepared with your full name, date of birth, and Medical Record Number (MRN). Without the MRN, you will need to contact a hospital representative. These fields will be compared and must match the information in our hospital record.

Completed successfully, you will see the screen above stating an email was sent to the unique email you listed in the self enrollment. Check your email to access your account.

Email Exchanges

In order to protect your personal information, Parkview Medical Center (PMC) discourages the use of regular email for patient correspondence. Email messages originated by you, our patient, are not sent in a secured, encrypted format which may result in your personal information being exposed to others. PMC employs a secure email solution for all emails originated by PMC. This does not apply to emails sent to PMC from a regular personal email account.
