I appreciate the whole staff whenever I call, OR an office visit is made.
December 08, 2021
All was well ..
December 03, 2021
I had a procedure done at the outpatient surgery by Dr. Chapman off of the Boulevard and it was very clean and very quick and easy but my care was greatly delayed by weeks because the Parkview operating rooms are closed because supposedly because of a patient covid, patient load at the hospital, even though it was an outpatient surgery, which I don't understand. The only thing that could have been done better is you need to open the operating room, so we can just get our procedures done, and get back to work. But Dr. Chapman's office, his crew, his surgical crew, all spectacular. Wonderful service. We just need to be opened up the operating rooms. Let them do their jobs and let us all get back to work. Thank you so much.

I was repressed. I was impressed with Dr. Chapman. He was incredibly cordial. He took his time. He explain things clearly and completely. He seemed to look at my whole body. He knew the information about general information. About my my case, prior to my meeting, for the first time. I'm very impressed. You could not have done a better job. I've just been really hopeful for the future result. He was a was lovely to deal with and I really would have no problem going back to him for anything that he was covered. Thank you very much.

I would not change anything. It was an excellent experience and I look forward to future treatments/appointments.

The whole office visit was one that we were... my daughter and I were both extremely impressed with... We would recommend it to any of our family and friends, highly recommend it. Thank you.

No changes - this was a good experience. Dr Chapman was a good listener and gave a thorough explanation of my situation. I would highly recommend Dr Chapman!!

September 08, 2021
Nothing good job all around
August 02, 2021
I felt Dr. Chapman was not compassionate whatsoever to my needs. He made me feel like the only option would be injections. I have been taking the medication I'm on for the past 10 years now. I know how to function with it. I do very well with it and it's the only thing that seems to help me, and in his opinion, he really didn't care what I had to say.

Have the address more prominent. Did not know I was in the right place until I went into a door and asked for the place I was looking for.
July 26, 2021
With a... my visit with Dr. Jack was very pleasant and very informative and I liked very much the way the people received us and and helped us fill out our paperwork and all that sort of thing. I'm really pleased with the office. I appreciate that.
July 19, 2021
I thought maybe that, I didn't think I...that he gave me enough time or he didn't explain things well enough about my situation. He said my insurance will pay for two shots, but they'll call me and let me know, set up appointment for my shots. So, I don't know. I was so stressed out because that was the day that my cab driver, they were short of drivers and I kind of got there late and I was stressed out and they were, I don't know, like I Tell you, it is...I'll probably know more next time I see and talk to him the next time, when I'm not so stressed out because I was like an or something late, so I ran into the lunch hour. So, okay, I guess I'll have to wait and see you and I'll see when they call me and make an appointment for my shots.

April 28, 2021
The doctor was fantastic. He was so gentle and relaxing that I relaxed and he knew right away. What was bothering me. He explained to me what he could do to get rid of it. Kind of scared, but he's he's he's great. Thank you.
April 21, 2021

April 13, 2021
Nothing! Everything was great
April 05, 2021
You know guys, I don't know Dr. Chapman from anyone else on the street. I use part of Parkview and I think he's probably a pretty good doctor because of that but you know having to wait 3 - 4 months to be able to get in to see the doctor is unacceptable in my perspective and from my perspective and and I just don't appreciate that. So I think if you could get your patients in a more timely fashion, that would be fabulous. You know, everybody has to be careful because of covid, I get that but we don't have to you know, why should we have to put our health concerns on hold? This is a doctor somebody who should be treating people that are sick and who are in firm, so I would think that you'd want to see his patients much sooner than 4 months. That's all.
March 22, 2021
They are professional to the enth degree. It takes a long time, I'm worth it when it's mine
March 17, 2021
The staff at this office are THE worst. So rude and short. I had an appointment with the doc and came as scheduled but when I got there, they said they called to cancel it. I never received a message or notice that the appointment needed to be rescheduled. I argued with them, rightfully so, and they were extremely rude back. Poor staffing.
March 16, 2021
Respectful knowledgeable, concerned
March 10, 2021
Very understanding
March 08, 2021
I can not think of anything that would have made it go smoother.
March 02, 2021
I would greatly appreciate Improvement on the waiting in the wait room. I waited 45 minutes to get to the examining room. And then I waited another 45 minutes and when you're in a lot of pain that's hard. And I'm done, thank you.
March 01, 2021
When I was visiting with your doctor, everything seems very fine. But when you call right after a first appointment, you don't really have an accurate way to answer, because you don't know that much about your doctor after the first visit.
March 01, 2021
I had to wait in the waiting room. For 15 minutes, which is not bad. My appointment was at one time the doctor didn't see me until an hour and 10 minutes after my appointment time. That's the only problem I have.
February 10, 2021
Everything was great
February 08, 2021
Dr. Chapman is always incredibly always listen offers answers that I can use and then Antonette at the front office is in equally incredible. It's always a pleasure to see both of them.
February 02, 2021
