Biography

Dr. Hardik Doshi is board certified by the American Board of Psychiatry and Neurology in both Neurology and Epileptology. He is practicing with Parkview Medical Group Neurology in Pueblo, CO, where he serves as the only board certified epilepsy doctor in the region and the Medical Director of Parkview Medical Center’s Stroke Program. He is a firm believer in ongoing education; therefore, it is not unusual to see him working with medical students and residents.


Dr. Doshi’s commitment to excellent patient care is evident in his extensive training. After completing his Neurology residency training he pursued fellowship training in epilepsy. During his fellowship he trained in neurocritical care EEG monitoring, intra-cranial EEG recording during epilepsy surgery, mapping of eloquent cortex, determining location of electrodes in the brain, WADA testing for language and memory, and importantly, epilepsy monitoring unit training. As an Epileptologist, Dr. Doshi immediately established an Epilepsy Monitoring Unit (EMU), the only established EMU in Southern Colorado, upon arrival to Parkview Medical Center in order to best serve his patients. He works closely with Denver Health and University of Colorado Anschutz Medical Center, both of which are Level IV epilepsy centers for surgical epilepsy patients.

What is an Epilepsy Monitoring Unit?
This is a specialized unit for epilepsy where patients are admitted for a duration of 4-5 days for continuous EEG recording along with video monitoring. Numerous studies have shown that routine (20-30minutes) EEGs are not reliable for diagnosis of epilepsy.

EMU can be offered for various reasons:
1) Diagnostic purpose: one of the most common reason patients do not respond to epilepsy drugs is that they do not have epilepsy to begin with and have been falsely diagnosed and treated all along.


2) Management of medications: rapid switch over to another medication in a safe setting, weaning off medications, etc.


3) Pre-surgical evaluation: once seizures are not controlled with two medications, then either (a) the diagnosis of epilepsy is wrong or (b) the patient has severe epilepsy and a 3rd medication will only offer <5% chance of seizure-freedom. In this case, evaluation for epilepsy surgery is needed as this offers about a 50% chance of seizure freedom and is usually safe if done at specialized centers. Other testing that may be needed include: PET scan, SPECT scan, functional MRIs, epilepsy protocol MRIs, neuropsychological evaluation, WADA testing, etc. The patient’s case is then presented at “Epilepsy surgery conference”, held at specialized centers once a month where more than 20-30 health care professionals participate to decide whether the patient will be a potential surgical candidate. This includes pediatric and adult board-certified epileptologists, neurosurgeons trained in epilepsy surgeries, neuropathologists, neuropsychologists, epilepsy nurses, and other care team professionals.
