Dr. Jack A. Chapman is a dual Diplomate of the American Boards of Anesthesiology and Pain Management with over twenty years of experience of providing outpatient treatment. He is a graduate of Baylor College of Medicine, completed an anesthesia residency at the University of Texas Medical Branch in Galveston, and then pursued fellowship training in cardiovascular and obstetrical anesthesia. His diverse training and vast experience in treating both acute and chronic pain allow him to offer patients a variety of treatment options to best address their unique symptoms and help them to achieve a greater quality of life. Dr. Chapman believes in thorough evaluations, transparent discussions of options and a commitment to follow up care to ensure the best outcome possible for each of his patients. 

Dr. Chapman provides office evaluation and management of all acute and chronic pain including but not limited to:

  • Back and neck pain
  • Headaches
  • Cancer related pain
  • Diabetic foot pain
  • Pain associated with Shingles
  • Pain following surgery
  • Progressively worsening arm and leg pain from injury or surgery

Dr. Chapman offers the following treatments for his patients:

  • Kyphoplasty
  • Percutaneous vertebroplasty
  • Selective nerve root blocks
  • Percutaneous intradiscal electrothermal annualoplasty (IDET)
  • Simple or complex implanted spinal cord and peripheral nerve stimulation systems
  • Dorsal Root Ganglion (DRG) stimulators
  • Implanted intrathecal pain pumps for severe intractable pain
  • Trigger point injections, Botox, and Myobloc injections
  • Epidural steroid injections (cervical, thoracic, lumbar)
  • Transforaminal steroid injections (cervical, thoracic, lumbar)
  • Interlaminar epidural steroid injections (cervical, thoracic, lumbar)
  • Facet and sacroiliac joint injections (cervical, thoracic, lumbar)
  • Facet and sacroiliac joint radiofrequency neurotomy
  • Percutaneous nucleoplasty and discectomy
  • Discography
  • Sympathetic nerve blocks 
  • Intra-articular hip injection

I felt Dr. Chapman was not compassionate whatsoever to my needs. He made me feel like the only option would be injections. I have been taking the medication I'm on for the past 10 years now. I know how to function with it. I do very well with it and it's the only thing that seems to help me, and in his opinion, he really didn't care what I had to say.

With a... my visit with Dr. Jack was very pleasant and very informative and I liked very much the way the people received us and and helped us fill out our paperwork and all that sort of thing. I'm really pleased with the office. I appreciate that.

I thought maybe that, I didn't think I...that he gave me enough time or he didn't explain things well enough about my situation. He said my insurance will pay for two shots, but they'll call me and let me know, set up appointment for my shots. So, I don't know. I was so stressed out because that was the day that my cab driver, they were short of drivers and I kind of got there late and I was stressed out and they were, I don't know, like I Tell you, it is...I'll probably know more next time I see and talk to him the next time, when I'm not so stressed out because I was like an or something late, so I ran into the lunch hour. So, okay, I guess I'll have to wait and see you and I'll see when they call me and make an appointment for my shots.

The only possible thing was if I had received prior authorization on prescription but it was approved and I was able to get prescription on that Saturday.

The doctor was fantastic. He was so gentle and relaxing that I relaxed and he knew right away. What was bothering me. He explained to me what he could do to get rid of it. Kind of scared, but he's he's he's great. Thank you.

You know guys, I don't know Dr. Chapman from anyone else on the street. I use part of Parkview and I think he's probably a pretty good doctor because of that but you know having to wait 3 - 4 months to be able to get in to see the doctor is unacceptable in my perspective and from my perspective and and I just don't appreciate that. So I think if you could get your patients in a more timely fashion, that would be fabulous. You know, everybody has to be careful because of covid, I get that but we don't have to you know, why should we have to put our health concerns on hold? This is a doctor somebody who should be treating people that are sick and who are in firm, so I would think that you'd want to see his patients much sooner than 4 months. That's all.

The staff at this office are THE worst. So rude and short. I had an appointment with the doc and came as scheduled but when I got there, they said they called to cancel it. I never received a message or notice that the appointment needed to be rescheduled. I argued with them, rightfully so, and they were extremely rude back. Poor staffing.

I would greatly appreciate Improvement on the waiting in the wait room. I waited 45 minutes to get to the examining room. And then I waited another 45 minutes and when you're in a lot of pain that's hard. And I'm done, thank you.

When I was visiting with your doctor, everything seems very fine. But when you call right after a first appointment, you don't really have an accurate way to answer, because you don't know that much about your doctor after the first visit.

I had to wait in the waiting room. For 15 minutes, which is not bad. My appointment was at one time the doctor didn't see me until an hour and 10 minutes after my appointment time. That's the only problem I have.

This visit was excellent everything about it was the doctors going to be a few minutes late. They called me told me that so it was excellent this time in the past I have had trouble getting a call back, but my experience this time was excellent and my overall I'm just very appreciative of the care from Dr. Chapman. This was a telephone visit due to Covid restrictions. Dr. Chapman did an excellent job answering the questions I his office staff will be sending more information to evaluate. He was very thorough and spent a good amount of time discussing options with me. I was very pleased with the meeting and feel like I may be able to get some help with my nerve pain.

I think that I would have liked to have been seen in person and when he was reading my MRI, he could have showed me. You know, what what the problems were with my neck and back and that's the only two things.

Would be best have a live visit with the doctor but with the covid going on we have to do it on the phone, but still would be better if we were live.

Dr. Chapman has been excellent, as far as taking care of me. The staff has had several problems getting my new pain pump figured out. It's a different pain pump then they're use to using. And I think they need a little bit of in house education on what they're doing. They try hard, but you know, they're ... they're .... they're good about trying. But they're just not getting it. Again, I'm totally happy with Dr. Chapman. We just need to get this pain pump worked out. They don't really know how to use it. They tried very hard Wednesday to get it going and they couldn't. So, I've been kind of in a dilemma. I'm going back Tuesday. Let's see if we can get this resolved.

Waited over 1hour for appointment over the assigned time. I know problems and concerns from previous patients sometimes take more than the patient slotted time. I hope this is not the standard. Perhaps if the staff would let the waiting patient know there is a delay, I would have felt better.
