Biography
Dr. Jack A. Chapman is a dual Diplomate of the American Boards of Anesthesiology and Pain Management with over twenty years of experience of providing outpatient treatment. He is a graduate of Baylor College of Medicine, completed an anesthesia residency at the University of Texas Medical Branch in Galveston, and then pursued fellowship training in cardiovascular and obstetrical anesthesia. His diverse training and vast experience in treating both acute and chronic pain allow him to offer patients a variety of treatment options to best address their unique symptoms and help them to achieve a greater quality of life. Dr. Chapman believes in thorough evaluations, transparent discussions of options and a commitment to follow up care to ensure the best outcome possible for each of his patients.
Dr. Chapman provides office evaluation and management of all acute and chronic pain including but not limited to:
- Back and neck pain

- Cancer related pain
- Diabetic foot pain
- Pain associated with Shingles
- Pain following surgery
- Progressively worsening arm and leg pain from injury or surgery

- Interlaminar epidural steroid injections (cervical, thoracic, lumbar)
- Facet and sacroiliac joint injections (cervical, thoracic, lumbar)
- Facet and sacroiliac joint radiofrequency neurotomy
- Percutaneous nucleoplasty and discectomy
